The maintenance device had been inserted into a pressure tube in the reactor core, and then the maintenance device accidently I'm quoting here burned a hole through the pressure tube in the calandria tube that encases it. It took, I believe, the better part of 24 hours to bring that under control and resulted in it being off-line.
